Virtual Ward Staff Nurses
Job summary
We are looking for enthusiastic, innovative and engaging Band 5 NMC Registered Nurses who have a passion for the delivery of high quality patient care within the community to join our Acute Care at Home - Virtual Wards.
Our Virtual Ward is a new and innovative service which offers face to face acute care in the patient's own home. The Virtual Ward has an agreed admission criteria which works in partnership with supporting care services and the aim of the service is to reduce length of acute hospital stay and/or avoid hospital admission; this leads to quicker recovery and reduces disruption to patients', and their carers' lives.
